PRE – CONSULTATION LIFESTYLE QUESTIONAIRE
This is emailed to you and returned shortly before your initial consultation

INITIAL CONSULTATION
60 minutes and can be face to face or by telephone

2 FOLLOW UP CONSULTATIONS
30 minutes each, usually 4 and 8 weeks after initial consultation

ONGOING ADVICE AND SUPPORT
The facility to email questions to Stuart during and after the program is complete
